No annual fee and no account maintenance fee. Check the issuance fee separately — a free card you paid 100 USD to receive is not free, and some cards charge yearly under a name other than "annual fee".

Every fee below cites the issuer’s own documentation and the date we read it; anything the issuer does not publish is marked as such rather than estimated.
